Every clinician dealing with acid-base abnormalities is familiar with the Henderson-Hasselbalch equation, probably the most famous equation in biology. Unfortunately, most clinicians are not aware of the limitations of this equation: It contains a number of anomalies and is more descriptive than mechanistic.

The Henderson-Hasselbalch equation has proved invaluable in aiding the understanding of mammalian acid-base physiology and is routinely used in the treatment of acid-base abnormalities. It was evident as early as 1922 that factors other than Pco2, [HCO3−], pK1′, and S influence plasma pH.16 In 1925, pK1′ (and therefore pH) was shown to be influenced by ionic strength (μ)17 and temperature,18 the ΔpK1′/ΔT being −0.005 units/° C over a temperature range of 20° to 38° C. It had been expected that pK1′, like all equilibrium constants based on molalities, would be altered by changes in ionic strength and temperature. It was unexpectedly found, however, that the measured ΔpH/ΔT of plasma was −0.015 to −0.020 units/° C, a value three to four times the −0.005 units/° C predicted by pK1′.6 This finding shows that the Henderson-Hasselbalch equation cannot be accurately applied to mammalian blood cooled in vitro or to poikilothermic animals.

Determination of accurate pK1′ values for plasma has been more problematic, because the experimental value for pK1′ (the apparent dissociation constant) in plasma differs marginally from the value obtained in aqueous nonplasma solutions. A number of studies have demonstrated that the value for pK1′ in plasma is influenced by pH, protein concentration, and sodium concentration.19–22 Although these observations troubled most investigators, the value for pK1′ in plasma is routinely adjusted through the use of nomograms, tables, or polynomial equations to account for changes in plasma pH, protein concentration, and sodium concentration. The mechanistic basis for these adjustments is currently unknown.

The value for pK1′ at an ionic strength of 0.16 (mammalian extracellular fluid) is obtained from the sum of pKs (the apparent dissociation constant) (6.029 at 37° C)19 and the negative logarithm of the activity coefficient of the hydrogen ion (0.091),22 producing a value of 6.120 at 37° C. The value for pK1′ should be corrected for temperature and ionic strength only when applied to biological fluids. The value used for S in plasma at 37° C is 0.0307 mm Hg−1; S varies with ionic strength, temperature, and protein concentration, and accurate values are available for mammalian plasma.23

Because the Henderson-Hasselbalch equation does not satisfactorily account for the temperature dependence of plasma pH or for the apparent dependence of pK1′ in plasma on pH, protein concentration, and sodium concentration, the approach can be accurately applied to only mammalian blood at normal body temperatures and approximately normal pH, protein concentration, and sodium concentration. The empirical nature of the adjustments to the value of pK1′ in plasma suggests that the Henderson-Hasselbalch equation is more descriptive than mechanistic. A more mechanistic approach is therefore desirable, and strong ion difference theory provides such a model.
